To assist persons who wish to apply for the Ontario Disability Support Program (ODSP) with assistance under OW until the applicant’s eligibility for ODSP is determined.
Anyone has the right to apply for the ODSP if they believe themselves to be “a person with a disability” or a member of a prescribed class as set out in the ODSP legislation
The likelihood of an applicant or participant being found eligible for ODSP is not to be taken into consideration when the applicant or participant indicates that he/she wishes to apply for ODSP.
OW Directive 2.4 provides details outlining ODSP referrals. Refer to this Directive for details relating to eligibility, assets, referral process, monitoring etc.
The Disability Determination Package (DDP) consists of a Health Status Report, an Activities of Daily Living Report , a Self Report and a Consent to Release of Medical Information.
If an applicant or participant requests a referral to ODSP the DAU will send out the DDP as defined during the latter part of this section.
