Re: 2023-24 - Base Funding for Alternate Level of Care (ALC)

Ontario Health is pleased to advise that Manitoulin-Sudbury District Services Board will receive base funding in the amount of $369,806 in fiscal year 2023-24 for the above-named Project or Program.

This Funding will support the continuation of the ALC initiatives started in 2022-23 in the Home and Community Care Project category.
 

Project Title Amount Short Description
Community Paramedicine $369,806 This funding will support additional Community Paramedics to enhance the frequency of home visits, additional health and wellness clinics, additional in home assessments, testing and follow- 
up. The delivery of this programming will address admission diversion and increase discharge support.

Details of the funding and the terms and conditions on which it will be provided are set out in the attached Appendix A and Schedules, if any.

In accordance with Section 22 of the Connecting Care Act, 2019, Ontario Health hereby gives notice that, subject to your organization’s agreement, it proposes to amend the Multi-Sector Service Accountability Agreement (MSAA) between Manitoulin-Sudbury DSSAB and Ontario Health with effect as of the date this letter is signed back by your organization. To the extent that there are any conflicts between the MSAA and this letter, the terms and conditions in this letter, including Appendix A and Schedules, if any, will govern. All other terms and conditions in the MSAA will remain the same.

Financial records of this allocation are to be maintained for year-end evaluation and settlement in accordance with the MSAA and/or Ministry policy.
 
Please indicate your organization’s acceptance of the MSAA amendment set out in this letter, including Appendices and Schedules, if any, by signing below and returning the signed version of this entire letter via email to oh-ne-finance@ontariohealth.ca within 10 business days of the date of this letter.
